Commercial Insurance
Cassopolis Family Clinic Network (CFCN) accepts many commercial insurances. Please check with your insurance company to see if CFCN is within your provider network, or call us at (269) 445-3874. In most cases CFCN will file a claim with your insurance company for payment. Co-payments are due at the time of visit.

CFCN is accepting new Medicare patients. CFCN will bill Medicare and secondary insurances. If there is a balance after Medicare and/or secondary insurance payments, patients will receive a statement from CFCN.

CFCN is accepting new Medicaid patients, including those with straight Medicaid, and those covered by the following Medicaid Health Plans:

    • McLaren Health Plan
    • Meridian Health Plan
    • United Healthcare Community Plan

Uninsured? We can help!

      • Healthy Michigan Plan provides coverage for people ages 19-64, have income at or below 133% of the federal poverty level and are not enrolled in or qualify for Medicaid or Medicare. We can help you apply.
      • Breast & Cervical Cancer Control Program provides coverage for breast and cervical cancer screenings for women ages  40 – 64 based on income requirements. For women under 40, diagnostic services are available.  We can help you apply.
      • Healthy Kids Children ages 0-18 may qualify for Medicaid coverage. We can help you apply.
      • MOMs Medicaid coverage for pregnant women. We can help you apply.
      • Sliding Fee Discount Program Discounts are available to patients who are uninsured or underinsured based on current Federal Poverty Guidelines for household size and gross income. To qualify, patients must complete an application and provide proof of income. Patients must meet with a Community Health Worker prior to their appointment to determine eligibility and the amount of discount. Nominal fees for services are due at the time of the visit.  Patients must reapply annually and provide proof of income.

CFCN is a certified agency to assist with health insurance and Medicaid enrollment under the Affordable Care Act.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
