How Do I Schedule an Appointment?

Cassopolis Family Clinic Network (CFCN) welcomes new patients and strives to schedule all patients, new or existing, in a timely manner.

To make an appointment, call the location you wish to go to.  The receptionist will inform you of what to bring with you to your appointment, for example, medication list, current insurance card, identification card, proof of income, etc.

CFCN schedules appointments with an open access concept.  A select number of appointments are open each day to allow for same day appointments.  Appointments fill up quickly and it is encouraged that patients call ahead to schedule appointments with their provider.  Patients who walk in without an appointment will be given an appointment time.  CFCN staff continues to do their best to have each patient seen as quickly as possible.

How Do I pay my bill?

Co-pays are due at the time of service.  CFCN accepts cash, check, Mastercard, Discover, and Visa.  If you have questions regarding your bill, please call the business office at (269) 262-4267.

How do I schedule transportation to the Health Center?

If transportation assistance is needed for your appointments, please call and ask to speak to a Community Health Worker one week prior to the day of the scheduled appointment.
