A Community Health Worker (CHW) is a front-line public health worker whose goal is to improve patient care by addressing social determinants of health. CHWs act as liaisons between the community and the healthcare system by providing additional resources and support to patients outside of the care from their healthcare team.

Assistance with Referrals
Community Health Workers play a vital role in connecting individuals to appropriate, trusted referral programs that help them access the resources they need. Referrals include:
- Medical ID Bracelet sponsorship
- Medical alert Pendant Sponsorship
- Medical needs forms for DHHS clients
- BCCCNP for uninsured women who meet requirements for free mammogram or cervical screenings
