Social Worker Joins Cassopolis Family Clinic Network

Cassopolis, MI, October 30, 2015– Mitchell B. Hannafius, Licensed Masters Social Worker, has joined the staff of Cassopolis Family Clinic Network (CFCN).

Hannafius educational background consists of associate degree from Bay de Noc Community College in Escanaba, Michigan, bachelor of Social Work degree from Northern Michigan University in Marquette, and master of Social Work degree from Western Michigan University.  Prior to his employment at CFCN, Hannafius was the owner and licensed therapist of Jericho Counseling Center of Niles.  Hannafius shares his time at Ranger Wellness Center located in Ross Beatty Jr. / Sr. High School and Cassopolis Family Clinic.

Senator Proos Visits Cassopolis Family Clinic

Cassopolis, MI, September 1, 2015 – Senator John Proos visited Cassopolis Family Clinic on Friday, August 21st to tour the facility and learn more about how the Community Health Center is increasing access to affordable health care for the residents of Cass and southeast Berrien County.

Senator Proos and District Representative Adam Mensinger spent over an hour with CFC leadership during their recent visit to the new facility.  Proos first took a tour of the facility stopping at the pharmacy, primary care, dental and behavioral health. On his tour he was able to speak with Community Health Worker (CHW), Stacy Lozada, to better understand how the CHW Program helps to assist patients with every day needs such as transportation and medication management.

Next, Proos sat down with Mary Geegan Middleton, CEO, to discuss the future of Cassopolis Family Clinic Network and its plans to increase access to primary care in the Niles community.     “I am proud of the great work that is being done by the Cassopolis Family Clinic every day in our Southwest Michigan community,” said Proos.  “It was wonderful to see firsthand the way that the clinic is providing necessary healthcare services to individuals and families that might otherwise go without.”

Cassopolis, MI, August 27, 2015 – Cassopolis Family Clinic was honored to host a visit by the Director of Michigan Department of Health and Human Services (MDHHS) Nick Lyon on Friday, August 14th. It was Director Lyon’s first visit to Cassopolis Family Clinic since he was selected to serve as MDHHS Director by Governor Snyder in January 2015.
Director Lyon’s visit included a tour of the Health Center, during which he met with administrative and support staff to discuss the overall operation and uniqueness of the Community Health Center Program. Shirley Lee, Outreach and Enrollment Specialist, explained how Certified Outreach and Enrollment staff take time to meet privately with uninsured patients to discuss their health care coverage options, and assist them in the application process for Medicaid or the Health Care Marketplace.
Director Lyon expressed appreciation for Outreach and Enrollment Specialists, such as Shirley, who are committed to helping the uninsured find health care coverage. Director Lyon oversees the Michigan Medicaid Program and was instrumental in the implementation and management of the Healthy Michigan Plan.
“Community Health Centers like Cassopolis Family Clinic play an important role in improving the health and wellness of Michigan residents,” said Nick Lyon, director of the MDHHS. “Working together we ensure that more Michiganders have access to the care they need to lead healthy, productive lives.”

Cass Native Joins Cassopolis Family Clinic

Cassopolis, MI, August 18, 2015– Tabitha Pollard, Family Nurse Practitioner, has joined the medical staff of the Cassopolis Family Clinic Network (CFCN).

Pollard, a 2006 Ross Beatty Jr./Sr. High School graduate, received her Associate of Science Degree in Nursing from Southwestern Michigan College, Bachelor of Science in Nursing and Master of Science Family Nurse Practitioner from Goshen College in Goshen, Indiana. She is Board Certified by the American Academy of Nurse Practitioners.

“At a young age I knew I wanted to become a nurse. Then, wanting to be more autonomous in caring for my patients, continued my education to become a Family Nurse Practitioner,” said Pollard. “With this opportunity, I can be an instrumental in providing health and wellness services for families I grew up with. Having that ability is gratifying.”

A Word From the CEO

Cassopolis – August 8, 2015 – Even as more Americans have health insurance coverage these days, a serious healthcare problem persists nationwide: a shortage of places to go to get the primary and preventive care services that everyone needs. A key part of the solution lies in our nation’s Community Health Centers, which have provided quality, affordable care for the past five decades, and are increasingly becoming the go-to medical home for growing numbers of working Americans.

What began as a modest demonstration program in the mid-1960s has evolved 50 years later into the largest and most successful primary care system in the country. Since their inception, health centers have demonstrated impressive results in reducing infant mortality, improving immunizations rates, developing programs for early screening and treatment of cancer, and managing chronic conditions such as obesity and diabetes. Today, health centers serve more than 23 million Americans, including about seven million children and 260,000 veterans in more than 9,200 communities across the country.

Why does the demand for health center services continue to grow?  One reason is that they are good at what they do.  The care health centers provide is as good or even better that what one finds in a private physician practice. Health center patients report satisfaction levels of nearly 100 percent. Also, health center services are conveniently offered under one roof and may include dental, pharmacy, mental health and substance abuse counseling, vision, pediatric, geriatric, OB-GYN, and much more.

There are health centers like ours across the country which save the U.S. healthcare system more than $24 billion every year in reduced overall costs from preventable hospitalizations and avoidable emergency room visits—a figure that dwarfs the federal investments made in their operations. Sustained public investment is critical. Without it, millions of patients could lose access to care and trigger a backlash of increased healthcare costs at a time when 62 million people across the country are already struggling with little or no access to affordable healthcare.

Health centers averted a massive cut to their funding earlier this year thanks to action by Members of Congress.  We are grateful to Senators Debbie Stabenow and Gary Peters, who, along with Congressman Fred Upton, supported continued federal funding of the health center program. Without their leadership, hundreds of families in our community would have lost access to affordable healthcare at Cassopolis Family Clinic Network.  We are grateful for their support and are using National Health Center Week 2015 (August 9-15) to thank them for their vote. Because of their efforts, health centers can continue to serve people and communities in need.
