About CFCN – Cassopolis Family Clinic Network (CFCN), a 501(c) 3 non-profit organization, is an independent, Federally Qualified Health Center (FQHC) providing primary care and support services for the communities of Cass and southeast Berrien Counties, Michigan. A Patient-Centered Medical Home, CFCN provides services for individuals who are uninsured, underinsured, covered by Medicare and Michigan Medicaid, along with most commercial insurances. CFCN began serving the community in 1964. Today, the health center cares for nearly 13,000 patients annually between 3 locations in Cassopolis and Niles.  Governed by a volunteer Board of Directors, CFCN remains committed to growing and changing to meet the needs of the residents of Cass and southeast Berrien Counties.

Statement of Nonpartisanship  Cassopolis Family Clinic Network (CFCN) may take positions on or advocate with regard to public policy issues pertinent to its mission, consistent with the requirements of applicable Federal and state law. However, CFCN conducts all of its activities in an entirely nonpartisan manner. CFCN may acknowledge the elected officials who support us, but we do not endorse, support, or oppose candidates for public office, nor do we intervene in any election on behalf of or in opposition to any candidate or political party.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
